Comparative characteristics of the complement system in patients with C3-glomerulopathy and atypical hemolytic uremic syndrome of chronic course who suffered an acute episode of thrombotic microangiopathy

چکیده

Aim. To compare changes in the complement system C3-glomerulopathy (C3-GP) and atypical hemolytic uremic syndrome (aHUS) after relief of an acute episode thrombotic microangiopathy.
Materials methods. The study included 8 patients diagnosed with C3-GP aHUS remission. blood levels components were determined: C3, C4, C3a, C5a, factor H (CFH), B (CFB), membrane-attacking complex (MAC), antibodies to C3b (anti-C3b-AT), level activity (CH50), content D (CFD) urine.
Results. C3 CH50 within reference range both groups, however, group they at lower limit, was significantly than group: 0.56 [0.44; 0.96] vs 1.37 [1.16; 2.52] (p=0.003). CFB increased detected but it 275.1 [222.1; 356.6] 438.7 [323.3; 449.3] (p=0.010). C5a MAC maximum group, 2 times higher that aHUS, these differences reached statistical significance 123 5556686 56031294 (p=0.036). CFH CFD their highest values group.
Conclusion. Alternative pathway activation signs present groups complement-mediated nephropathies, regardless stage disease. In C3-GP, alternative more pronounced microangiopathy.

C3 Glomerulopathy and Atypical Hemolytic Uremic Syndrome: Two Important Manifestations of Complement System Dysfunction

The advances in our understanding of the alternative pathway have emphasized that uncontrolled hyperactivity of this pathway causes 2 distinct disorders that adversely impact the kidney. In the so-called atypical hemolytic uremic syndrome (aHUS), renal dysfunction occurs along with thrombocytopenia, anemia, and target organ injury to multiple organs, most commonly the kidney. On the other hand,...

Thrombotic Microangiopathy with Complement Factor H Gene Mutations Unassociated with Atypical Hemolytic Uremic Syndrome

To the Editor, Atypical hemolytic uremic syndrome (aHUS) is a rare multigenic disorder characterized by thrombotic microangiopathy (TMA). Among the genes that are associated with aHUS, mutations in the complement factor H (CFH) gene are the most common genetic cause of the disease. Several specific gene mutations have been identified in patients with aHUS [1,2,3]. A 5-year-old boy was admitted ...

Postoperative Atypical Hemolytic Uremic Syndrome Associated with Complement C3 Mutation

Atypical hemolytic uremic syndrome (aHUS) can be distinguished from typical or Shiga-like toxin-induced HUS. The clinical outcome is unfavorable; up to 50% of affected patients progress to end-stage renal failure and 25% die during the acute phase. Multiple conditions have been associated with aHUS, including infections, drugs, autoimmune conditions, transplantation, pregnancy, and metabolic co...

Clinical evaluation of thrombotic microangiopathy: identification of patients with suspected atypical hemolytic uremic syndrome

Atypical hemolytic uremic syndrome (aHUS) is a rare genetic disorder caused by defective complement regulation resulting in thrombotic microangiopathy (TMA). Patients can present as children or adults. The syndrome consists of hemolytic anemia with schistocytosis, thrombocytopenia, significant renal damage, and/or other organ system dysfunction(s). Patients with aHUS may succumb to the complica...

Postpartum thrombotic microangiopathy revealed as atypical hemolytic uremic syndrome successfully treated with eculizumab: a case report

INTRODUCTION Differential diagnosis of thrombotic microangiopathies can be difficult. Atypical hemolytic uremic syndrome is a rare, life-threatening disease caused by uncontrolled chronic activation of alternative complement pathway, resulting in microvascular thrombosis, organ ischemia and damage. Prognosis is poor: up to 65 percent of patients require dialysis or have kidney damage of varying...
